How to fill in AS9102 Form 2 without errors

During the FAI approval of a turbine blade treated by chromic acid anodising, the inspector asks for a completed AS9102 Form 2. This document traces the raw materials, heat treatments and surface coatings. Simply forgetting the heat number of the aluminium cast is enough to stop the package from going to the customer.

Filling in AS9102 Form 2 for materials and processes

A practical guide to filling in Form 2: material traceability, special processes and certificates of conformance.

Field number AS9102 field title Required content Example entry
Field 5 Material or Process Name Name of the material or special process Aluminium 7075-T651
Field 6 Specification Number Applicable standard or customer specification AMS 4045 / EN 485
Field 7 Code Class, type or grade required by the specification T651
Field 8 Special Process Supplier Code Code of the subcontractor approved for the process Traitement Surface Aéro, F-1187
Field 9 Customer Approval Verification Subcontractor approved by the customer: yes, no or N/A Yes
Field 10 Certificate of Conformance Number Number of the material or process certificate CERT-MAT-2026-881
Field 11 Functional Test Procedure Number Functional test procedure, if there is one PE-HYD-300
Field 12 Acceptance Report Number Acceptance report of the test PV-2026-0412
Field 13 Comments Remarks that help the reader Heat number 4471

Fields 1 to 4 repeat at the top of the page the part number, name, serial number and report identifier, as on Form 1.

Raw material traceability and mill certificates

Form 2 (Product Accountability – Raw Material, Special Processes, Functional Testing) records the intrinsic quality of the metal or base component.

Every piece of stock going into manufacture (bar, sheet, forging) must be tied to its chemical and mechanical test certificate issued by the mill.

Field 5 holds the exact alloy grade. Field 6 gives the reference standard (for example AMS, ASTM or AIR).

The heat certificate number (Field 10) must match the number engraved or stamped on the blank before machining.

See our AS9102 report export features to follow how these certificates are built into the FAI package automatically.

Recording special processes and NADCAP approvals

Special processes (heat treatment, surface treatment, welding, non-destructive testing) call for great care.

Each process applied to the part must have its own line on Form 2.

If anodising is subcontracted, the provider’s code must appear in Field 8, and Field 9 confirms that it is one of the customer’s approved subcontractors (including NADCAP accreditation where required).

The certificate covering the anodic coating thickness and the salt spray test must be referenced in Field 10 and attached to the package.

Without proof that a special process is qualified, the conformity of the whole part is legally void.

A quality technician prepares Form 2 for a landing gear link machined from 300M steel, vacuum hardened and vacuum cadmium plated.

The raw material is covered by mill certificate MAT-300M-990.

  1. Line 1 (Material): Steel 300M / AMS 6419 / Certificate MAT-300M-990.
  2. Line 2 (Heat treatment): Hardened and tempered 1700 MPa / AMS 2759 / Certificate TT-2026-04.
  3. Line 3 (Coating): Vacuum cadmium plating 10 µm / AMS 2401 / Certificate TS-2026-112.
  4. Line 4 (NDT): Fluorescent penetrant, Class 4 / ASTM E1417 / Certificate NDT-8841.

Verdict: all the attached certificates comply with the assembly drawing specifications. Form 2 is approved for signature.

Classic pitfalls when filling in Form 2

The first pitfall is using assembly chemicals (adhesives, thread lockers, sealants) that aren’t listed. These consumables must appear on Form 2 with their expiry date and batch number.

The second is leaving out functional testing. If the drawing calls for a hydraulic proof test at 300 bar, the test procedure (Field 11) and the test report (Field 12) must be listed in the dedicated section of Form 2.

The third comes from material quantities that don’t add up against batch numbers. A 2 m² aluminium sheet can’t account for a thousand large housings without proof of further supplies.
